Even if the Coaches Association of Ontario has a screening process to help detect infections when there are symptoms, this screening process will not detect individuals who do not have any symptoms at the time of screening.

Are you currently experiencing any of these symptoms? Choose any/all that are new and not related to seasonal allergies or pre-existing medical conditions.

In the last 14 days, have you been in close physical contact with someone who tested positive for COVID-19? *
Close physical contact means: being less than 2 metres away in the same room, workspace, or area and/or living in the same home.
In the last 14 days, have you been in close physical contact with someone who is currently sick with new COVID-19 symptoms (like a cough, fever, or difficulty breathing)? *
Close physical contact means: being less than 2 metres away in the same room, workspace, or area and/or living in the same home.
In the last 14 days, have you been in close physical contact with someone who returned from outside of Canada in the last 2 weeks with new COVID-19 symptoms (like a cough, fever, or difficulty breathing)? *
Close physical contact means: being less than 2 metres away in the same room, workspace, or area and/or living in the same home.
Have you travelled outside of Canada in the last 14 days? *
This does not include essential workers who cross the Canada-US border regularly.
By clicking below and attending the workshop, you are attesting that you have NOT answered “Yes” to any of the questions above on the day of the workshop and that you acknowledge that Coaches Association of Ontario’s screening process cannot detect asymptomatic individuals. *
If No, please contact the CAO to discuss your options for attending another workshop in the future. There is NO PENALTY for withdrawing from the workshop at this time.
